Care Center
Northgate Care Center takes a Person Directed Care approach to
long-term care, encouraging the individuals we care for to make choices
in their health care, activities and meals. In addition to being a
24-hour skilled nursing center, Northgate Care Center also provides
physical, occupational and speech therapies, respite care, hospice
care, rehab-to-home, and free transportation to and from appointments
and activities. Our goal is to help each resident reach and maintain
their maximum level of independence by offering freedom of choices of
daily activities including: a variety of meal choices, open dining
hours, personalized room decor, and participation in individual and
group activities.

HOME CARE: ABCM Healthy Living Home Care operates as a
continuum of care through Northgate Care Center. This is a
non-medical, personal care and companionship service intended to assist
individuals with maintaining a safe environment so that they can live
